Documents Required

1. Transfer form to be duly filled and signed by both parties (Originals to be submitted)
2. Copy of NIC or Passports are accepted (Photo and Signature should be visible). For Minors: Photocopies of Birth Certificates AND their responsible party(ies) must sign on their behalf.
3. All Original Share Certificate(s): In case of lost or destroyed certificate(s), please click here.
4. Copy of Birth Certificate of Children and that of his/her guardian
5. Copy of NIC or Passports are accepted (Photo and Signature visible) of his/her guardian
6. Amendment form to be filled by the transferee to credit any onward dividend into his/her bank account
7. Copy of Bank Statement of the Transferee showing clearly the name and account number as proof
