Pertaining to this matter, a Judges’ Order is needed as per Article 394 of the Civil Code, in the above circumstances.

Documents Required

1. Transfer form to be duly filled and signed by both parties (Originals to be submitted)
2. Copy of NIC of parents /guardians or Passports are accepted (Photo and Signature should be visible)
3. All Original Share Certificate(s): In case of lost or destroyed certificate(s), please click here.
4. Copy of Birth Certificate of Child / Children
5. Amendment form to be filled by the transferee(s) to credit any onward dividend into its bank account
6. Copy of Bank Statement of the Transferee showing clearly the name and account number as proof
