The following documents have to be submitted to SIT Office together with the relevant forms (as detailed below) in original :

1. Copy of Affidavit(s) Registered At Mauritius
2. Copy of Death Certificate
3. Copy of NIC or Passports are accepted (Photo and Signature should be visible). FOR MINORS: Photocopies of Birth Certificates AND their responsible party(ies) must sign on thier behalf
4. All Original Share Certificate(s)In case of lost or destroyed certificate(s), please click here.
5. All uncashed Dividend Cheque(s) In case of lost cheque(s), please click here.
6. A Request letter addressed to the Directors, SIT, asking them to proceed with the transfer as has been agreed by the Succession. The letter must be signed by at least one heir.
7. Amendment form to be filled by the transferee, to credit any onward dividend into his/her bank account.
8. Copy of Bank Statement of Transferee showing clearly the name and account number as proof.
