In order to proceed with an amendment in your dividend disposal instructions, you are required to either submit a letter stating the dividend disposal instruction(s) or to fill in the attached “Amendment Form” and submit same in original together with the following documents:

Dividend Disposal instructions: Cheque

• Copy of NIC or Passports are accepted (Photo and Signature should be visible; if shares are held jointly, both NICs or Passports are required).

Dividend Disposal instructions: Bank Account

• Copy of NIC or Passports are accepted (Photo and Signature should be visible; if shares are held jointly, both NICs or Passports are required); and
• Copy of Bank Statement, showing clearly your name and bank account number.
