In order to proceed with an amendment in your existing bank particulars, you are required to either submit a letter stating the change to be effected to your existing bank particulars or to fill in the attached “Amendment Form”:

The Request Letter or Amendment Form should be submitted in original together with the following documents:

• Copy of NIC or Passports (Photo and Signature should be visible) are accepted; (if shares are held jointly, both NICs or Passports are required); and
• Copy of New Bank Statement, showing clearly your name and bank account number.
