You must fill in an “Indemnity - Amendment on Name Form” and submit same in original together with the following documents:

• Copy of NIC or Passports are accepted (Photo and Signature should be visible; if shares are held jointly, both NICs or Passports are required);
• Original Share Certificate(s) to be Amended
• In case of lost or destroyed certificate (s), please click here.
• Original Dividend Cheque(s) to be Amended (If applicable)
• In case of lost or destroyed dividend cheque (s), please click here.
• Copy of Marriage certificate (If applicable)
