If your option certificate has been lost or destroyed, an indemnity form would need to be duly filled in and the following documents would have to be submitted in order to obtain a duplicate certificate:

• Copy of NIC or Passports are accepted (Photo and Signature should be visible). FOR MINORS: Photocopies of Birth Certificates AND their responsible party(ies) must sign on thier behalf; and
• A fee of Rs 100/- would need to be paid for each lost certificate.
